Phone & Reception Intake Form

This form is completed by Client during or immediately after the initial phone consultation. The purpose is to capture the information needed to understand the clients requirements. Part of this process is to set up the client's dedicated phone lines, call routing, reception preferences, voicemail, and set up all devices. Once confirmed by the client, this document serves as the setup road map providing specifications to Amata's provisioning team.
